This form should only be used by Customers who have a defined Service Level Agreement with Internode. If you are unsure about whether you should use this form, please contact your Sales Representative or Account Manager.

Your Internode client number appears in the top right corner of your Internode invoices (eg. A123456). If you do not know your client number, please contact Internode for assistance.
